چکیده

A previously healthy five year-old boy was seen after intermittent strangulation attempts over the course of two days. Details of the event are vague due to lack of reliable adult witnesses. There was no indication of toxic exposure. General examination on arrival to the emergency department revealed an alert boy with bruising of the neck consistent with multiple strangulation attempts. There was no evidence of heart, kidney, or liver involvement to suggest systemic hypoperfusion or organ failure. Investigations done on arrival showed no acidosis, and a capillary blood gas 48 hours later revealed a normal carboxyhemoglobin. Although the initial neurologic exam was near normal, severe generalized dystonia developed over the next four weeks, requiring intrathecal baclofen treatment. No akinetic or rigid symptoms developed, however speech gradually

Childhood Laryngeal Dystonia Following Bilateral Globus Pallidus Abnormality: A Case Study and Review of Literature

Introduction:Dystonia is a disorder of movement caused by various etiologies. Laryngeal dystonia is caused by the spasm of laryngeal muscles. It is a disorder caused by vocal fold movement in which excessive adduction or abduction of the vocal folds occurs during speech. The pathophysiology of this type of dystonia is not fully known. Some researchers have suggested that basal ganglia structure...

Deep Brain Stimulation for Dystonia: Patient Selection, Surgical Tech- nique, and Programming

Deep brain stimulation (DBS) at the internal globus pallidus (GPi) is currently approved for the treatment of primary generalized and segmental dystonia. Younger age at surgery, shorter disease duration, and absence of fixed skeletal deformities correlate with a better response to stimulation. Patients with cervical dystonia may also improve. As a group, patients with secondary dystonias respon...

Dystonia with Brain Manganese Accumulation Resulting From SLC30A10 Mutations: A New Treatable Disorder

BACKGROUND The first gene causing early-onset generalized dystonia with brain manganese accumulation has recently been identified. Mutations in the SLC30A10 gene, encoding a manganese transporter, cause a syndrome of hepatic cirrhosis, dystonia, polycythemia, and hypermanganesemia. METHODS We present 10-year longitudinal clinical features, MRI data, and treatment response to chelation therapy...

Smoking Induced Worsening of Myoclonic Dystonia due to Haloperidol: A Case Report

Acute dystonia is a side effect of antipsychotic medication, and appears shortly after beginning treatment. The movement disorder is characterized by sustained muscle contractions that are typically slow, but rapid dystonia referred to as myoclonic dystonia has also been described. Cranial, pharyngeal and cervical muscles are generally affected causing fixation of the jaw, retrocollis, torticol...

[Pathophysiology in dystonia: functional anatomy of basal ganglia].

Clinical manifestation of dystonia features involuntary muscle contraction or spasm resulting in postural and movement disorders. Depending on the distribution and spreading pattern of dystonia, four patterns have been described, including focal, segmental, multifocal and generalized dystonia. Dystonia can be seen as one of additional clinical features in a more severe neurologic condition. Mos...
